If you previously drove a commercial motor vehicle, provide an additional seven years of employment history, for a total of ten years. Explain all employment gaps of more than one month. Start with your current or most recent employer and work backward. Include military experience. Attach additional sheets if needed.

I understand that I have the right to review information provided by current or previous employers, request correction of inaccurate information, and provide a rebuttal statement if an employer and I cannot agree on its accuracy.

I have been requested to provide a urine specimen for analysis for controlled substances. I understand that the analysis may indicate the presence of controlled substances in my system.

I have been instructed to report to a collection site within five business days after accepting an offer of employment. If I cannot report within that period, I must obtain approval from the person who sent the offer. An extension is not guaranteed.

Laboratory: Labcorp.

I understand that I may refuse testing. If I refuse, I understand that my conditional offer of employment will be withdrawn if I am a job applicant.

I consent to urine specimen collection and controlled-substance testing by the laboratory identified above and to release of the laboratory results to a Medical Review Officer and then to J L Barnett Trucking.

I understand that a positive controlled-substance test result will result in withdrawal of any job offer made to me if I am an applicant.
